las enfermeras del Dr. Martin Couney cuidaban a beb\u00e9s prematuros en peque\u00f1as incubadoras de vidrio en Coney Island, Nueva York, EE.\u00a0UU.\u00a0A lo largo de los a\u00f1os se han introducido e implementado numerosas t\u00e9cnicas y terapias, algunas simples y otras novedosas, ya que ahora se atiende a beb\u00e9s que son m\u00e1s peque\u00f1os y m\u00e1s fr\u00e1giles que nunca.<\/p><p>El conjunto cada vez m\u00e1s complejo de beb\u00e9s extremadamente prematuros trae consigo su propio conjunto de retos, entre ellos la dificultad de mantener su temperatura corporal.\u00a0La termorregulaci\u00f3n y la prevenci\u00f3n de la hipotermia ya ocupan un lugar destacado en la lista de prioridades en los departamentos de maternidad para los beb\u00e9s sanos a t\u00e9rmino, sin embargo, en el mismo entorno donde el beb\u00e9 de muy bajo peso al nacer (MBPN) es mucho m\u00e1s fr\u00e1gil y se ve afectado por la p\u00e9rdida de calor, es posible que deban adoptarse otras t\u00e9cnicas para evitar disminuciones peligrosas de la temperatura corporal central.<\/p><p>Las recomendaciones internacionales requieren que la temperatura corporal central de un reci\u00e9n nacido se mantenga dentro de un rango de 36,5\u00a0\u00b0C a 37,5\u00a0\u00b0C para proporcionar los mejores resultados para ese beb\u00e9 [<a href=\"https:\/\/www.ahajournals.org\/doi\/full\/10.1161\/CIR.0000000000000895\" target=\"_blank\" rel=\"noopener\">1<\/a>], y las unidades neonatales registran la temperatura de los beb\u00e9s a su ingreso como un marcador de la calidad de la atenci\u00f3n y del pron\u00f3stico del paciente.\u00a0Los estudios han demostrado que incluso un descenso de 1\u00a0\u00b0C en la temperatura de admisi\u00f3n aumenta el riesgo de sepsis en un reci\u00e9n nacido con MBPN en un 11\u00a0%, y el riesgo general de morbilidad aumenta en un 28\u00a0% [<a href=\"https:\/\/www.cochranelibrary.com\/cdsr\/doi\/10.1002\/14651858.CD004210.pub5\/full\" target=\"_blank\" rel=\"noopener\">2<\/a>,<a href=\"https:\/\/publications.aap.org\/pediatrics\/article-abstract\/119\/3\/e643\/70422\/Admission-Temperature-of-Low-Birth-Weight-Infants\" target=\"_blank\" rel=\"noopener\">3<\/a>].\u00a0Si combinamos estas estad\u00edsticas con la evidencia de que la temperatura corporal puede caer hasta 4\u00a0\u00b0C en los primeros 10 a 20 minutos de vida, si el reci\u00e9n nacido se queda sin protecci\u00f3n t\u00e9rmica [<a href=\"https:\/\/apps.who.int\/iris\/bitstream\/handle\/10665\/63986\/WHO?sequence=1\" target=\"_blank\" rel=\"noopener\">4<\/a>], el riesgo y los resultados asociados de hipotermia en los beb\u00e9s MBPN es muy grave.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-4c95b3e elementor-widget impacto positivo en situaciones en las que los beb\u00e9s se enfr\u00edan f\u00e1cilmente [<a href=\"https:\/\/www.cochranelibrary.com\/cdsr\/doi\/10.1002\/14651858.CD004210.pub5\/full\" target=\"_blank\" rel=\"noopener\">2<\/a>], (im\u00e1genes 1 y 2). Este blog se centrar\u00e1 espec\u00edficamente en la administraci\u00f3n de gases calientes cuando se brinda asistencia respiratoria al beb\u00e9 con MBPN, que es un requisito en muchos partos prematuros.<\/p><p>Los ensayos cl\u00ednicos, incluido el estudio de 2014 de Meyer\u00a0<em>et al<\/em>. [<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/abs\/pii\/S0022347614009007\" target=\"_blank\" rel=\"noopener\">5<\/a>], han demostrado los beneficios del uso de gases calientes a la hora de estabilizar a los beb\u00e9s con MBPN y de proporcionar ventilaci\u00f3n con presi\u00f3n positiva (VPP).\u00a0El concepto no es extra\u00f1o; despu\u00e9s de todo, es ampliamente aceptado y practicado que cualquier terapia respiratoria aguda terap\u00e9utica y de apoyo administrada en el entorno de la sala neonatal se realiza con calor activo y humidificaci\u00f3n.<\/p><p>La pr\u00e1ctica actual en muchos pa\u00edses es que los beb\u00e9s que requieren asistencia respiratoria despu\u00e9s del nacimiento la reciben en una cuna de plataforma (v\u00e9ase la imagen 2 anterior, por ejemplo) y a trav\u00e9s de un dispositivo de reanimaci\u00f3n con pieza en T integrada en el distribuidor vertical que se extiende desde la parte posterior de la cuna.\u00a0Esto permite que se inicie casi instant\u00e1neamente el apoyo requerido para el 10\u00a0% de los beb\u00e9s que no respiran cuando nacen [<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/abs\/pii\/S0300957221000678\" target=\"_blank\" rel=\"noopener\">6<\/a>], sin embargo, los gases administrados a trav\u00e9s de este sistema PPV provienen directamente de una fuente de gas entubada o de cilindros de gas y, en consecuencia, pueden describirse como gases \u00abfr\u00edos y secos\u00bb.\u00a0De hecho, una investigaci\u00f3n llevada a cabo en diferentes \u00e1reas de un hospital demostr\u00f3 que el ox\u00edgeno canalizado ten\u00eda una temperatura media de 23,3\u00a0\u00b0C y una humedad relativa (HR) de solo 2,1\u00a0%, mientras que el aire canalizado se midi\u00f3 a 23,4\u00a0\u00b0C con una HR de 5,4\u00a0% [<a href=\"https:\/\/onlinelibrary.wiley.com\/doi\/abs\/10.1111\/jpc.12393\" target=\"_blank\" rel=\"noopener\">7<\/a>], que son resultados bastante austeros.<\/p><p>Si tenemos al ya fr\u00e1gil beb\u00e9 MBPN que recibe VPP y estabilizaci\u00f3n con estos gases fr\u00edos y secos, es f\u00e1cil deducir que esto podr\u00eda llevar a que la temperatura corporal del beb\u00e9 descienda por debajo del rango recomendado.\u00a0Como m\u00ednimo, podemos esperar que, dado el impacto masivo de la conducci\u00f3n, convecci\u00f3n, radiaci\u00f3n y evaporaci\u00f3n [<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/abs\/pii\/S088421751530040X\" target=\"_blank\" rel=\"noopener\">8<\/a>] en estos beb\u00e9s prematuros, los gases fr\u00edos y secos no sean \u00fatiles para combatir la p\u00e9rdida de calor asociada con estas fuerzas termodin\u00e1micas, y aqu\u00ed el uso de gases calientes y humidificados podr\u00eda ser de gran ayuda.<\/p><p>Asimismo, la clave para el uso de gases calentados y humidificados es el beneficio que puede aportar para mejorar la funci\u00f3n pulmonar.
