avances positivos y mucha investigaci\u00f3n en los m\u00e9todos de tratamiento y la mejora de las tecnolog\u00edas de los productos sanitarios, lo que ha contribuido a forjar una funci\u00f3n espec\u00edfica e independiente para los profesionales sanitarios que antes se ocupaban de la \u00abpediatr\u00eda\u00bb como especialidad gen\u00e9rica. Los m\u00e9dicos y enfermeros que ejercen en la actualidad en el campo de la \u00abneonatolog\u00eda\u00bb han experimentado un cambio significativo, ya que el tama\u00f1o de sus pacientes se ha ido reduciendo progresivamente, mientras que los resultados esperados para estos beb\u00e9s no han dejado de mejorar. La termorregulaci\u00f3n y la prevenci\u00f3n de la hipotermia ocupan ya un lugar destacado en la lista de prioridades de los servicios de maternidad que atienden partos a t\u00e9rmino sanos. Sin embargo, dado que el beb\u00e9 de muy bajo peso al nacer (MBPN) es mucho m\u00e1s fr\u00e1gil y se ve m\u00e1s afectado por la p\u00e9rdida de calor, es posible que deban adoptarse otras t\u00e9cnicas para evitar peligrosas disminuciones de la temperatura corporal central. Las recomendaciones internacionales requieren que la temperatura corporal central de un reci\u00e9n nacido se mantenga dentro de un rango de 36,5\u00b0C a 37,5\u00b0C para proporcionar los mejores resultados para ese beb\u00e9 [1], y las unidades neonatales registran la temperatura de los beb\u00e9s a su ingreso como un marcador de la calidad de la atenci\u00f3n y del pron\u00f3stico del paciente. Los estudios han demostrado que incluso un descenso de 1\u00b0C en la temperatura de admisi\u00f3n aumenta el riesgo de sepsis en un reci\u00e9n nacido con MBPN en un 11%, y el riesgo general de morbilidad se incrementa en un 28% [2][3].<\/p><p>Si combinamos estas estad\u00edsticas con la evidencia de que la temperatura corporal puede caer hasta 4\u00b0C en los primeros 10 a 20 minutos de vida, llegamos a la conclusi\u00f3n de que, si el reci\u00e9n nacido se queda sin protecci\u00f3n t\u00e9rmica [4], el riesgo de hipotermia, con las consecuencias asociadas, en los beb\u00e9s de MBPN es muy grave. Dada esta base emp\u00edrica, los equipos perinatales realizan grandes esfuerzos para garantizar que los beb\u00e9s ingresados en cuidados neonatales registren temperaturas dentro del rango normal en el momento de la admisi\u00f3n. [5] han demostrado los beneficios del uso de gases calentados a la hora de estabilizar a los beb\u00e9s con MBPN y proporcionar ventilaci\u00f3n con presi\u00f3n positiva (VPP) o estabilizaci\u00f3n respiratoria. El beb\u00e9 con MBPN que puede beneficiarse en particular de la bolsa de polietileno o del calentador radiante tambi\u00e9n tiene muchas m\u00e1s probabilidades de requerir soporte respiratorio o de que se le ofrezca este inmediatamente despu\u00e9s del parto en comparaci\u00f3n con un beb\u00e9 nacido a t\u00e9rmino [6]. La pr\u00e1ctica actual en muchos pa\u00edses es que los beb\u00e9s que requieren asistencia respiratoria al nacer la reciban en una cuna de plataforma (v\u00e9ase la Imagen 2 anterior, por ejemplo) y a trav\u00e9s de un dispositivo de reanimaci\u00f3n con pieza en T integrada en el distribuidor vertical que se eleva desde la parte posterior de la cuna. Eso permite que los gases ricos en ox\u00edgeno y la presi\u00f3n positiva se inicien casi al instante para ese 10% de los beb\u00e9s que no respiran al nacer [7].<\/p><p>Sin embargo, los gases suministrados a trav\u00e9s de este sistema de VPP vienen directamente de una tuber\u00eda de gas o de bombonas de gas y, como consecuencia, se pueden describir como \u00abfr\u00edos y secos\u00bb. De hecho, una investigaci\u00f3n llevada a cabo en diferentes \u00e1reas de un hospital demostr\u00f3 que el ox\u00edgeno canalizado ten\u00eda una temperatura media de 23,3\u00b0C y una humedad relativa (HR) de solo un 2,1%, mientras que, para el aire canalizado, las mediciones fueron de 23,4\u00b0C con una HR del 5,4% [8], que son resultados bastante crudos. Si tenemos al ya fr\u00e1gil beb\u00e9 con MBPN que recibe VPP y estabilizaci\u00f3n con estos gases fr\u00edos y secos, es f\u00e1cil deducir que eso podr\u00eda hacer que la temperatura corporal del beb\u00e9 descienda por debajo del rango recomendado.<\/p><p>Como m\u00ednimo, podemos esperar que, dado el enorme impacto de la conducci\u00f3n, convecci\u00f3n, radiaci\u00f3n y evaporaci\u00f3n [9] en estos beb\u00e9s prematuros, los gases fr\u00edos y secos no sean \u00fatiles para combatir la p\u00e9rdida de calor asociada a estas fuerzas termodin\u00e1micas, y que el uso de gases calentados y humidificados pueda resultar de gran ayuda, como suger\u00edan Meyer et al. en el estudio de 2014.<\/p><p>Otro metaan\u00e1lisis de estudios adicionales que utilizaron reanimaci\u00f3n humidificada y climatizada para estabilizar a beb\u00e9s que necesitaron respiraci\u00f3n asistida al nacer incluy\u00f3 un total de 476 reci\u00e9n nacidos prematuros &lt;32 semanas de gestaci\u00f3n. El an\u00e1lisis mostr\u00f3 una reducci\u00f3n de la hipotermia al ingreso del 36% en el grupo calefactado y humidificado, lo que respalda su recomendaci\u00f3n de plantearse el calentamiento y la humidificaci\u00f3n de los gases inspirados durante la estabilizaci\u00f3n tras el parto y durante el transporte a la unidad neonatal [10].<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-943d151 elementor-widget elementor-widget-spacer\" data-id=\"943d151\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"spacer.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t<div class=\"elementor-spacer\">\n\t\t\t<div class=\"elementor-spacer-inner\"><\/div>\n\t\t<\/div>\n\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-b05780e elementor-widget elementor-widget-heading\" data-id=\"b05780e\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">Gas calefactado y humidificado<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-fa4b022 elementor-widget elementor-widget-spacer\" data-id=\"fa4b022\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"spacer.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t<div class=\"elementor-spacer\">\n\t\t\t<div class=\"elementor-spacer-inner\"><\/div>\n\t\t<\/div>\n\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-c245cf1 elementor-widget elementor-widget-text-editor\" data-id=\"c245cf1\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>El concepto de los gases humidificados en el entorno neonatal no es extra\u00f1o; al fin y al cabo, es una pr\u00e1ctica ampliamente aceptada y utilizada que cualquier terapia respiratoria aguda y de apoyo administrada en el entorno de una sala neonatal se lleve a cabo con calor y humidificaci\u00f3n activos. El tubo con cable calefactado y la c\u00e1mara de humidificaci\u00f3n son elementos familiares en la Unidad de Cuidados Intensivos Neonatales (UCIN) y pueden incluirse en un paquete para crear un circuito con una pieza en T humidificado y calefactado que suministrar\u00e1 presiones para respiraci\u00f3n con VPP en el mismo formato que una alternativa est\u00e1ndar no calefactada. El tubo con cable calefactado y la c\u00e1mara de humidificaci\u00f3n funcionan conjuntamente con un humidificador y calentador el\u00e9ctrico que suministra una fuente de calor y energ\u00eda al cable calentador, a\u00f1adiendo calor y vapor de agua al sistema a medida que el gas pasa a trav\u00e9s de la c\u00e1mara de humidificaci\u00f3n.
